Sick/Fit Note Certificates

Self Certification Sick/Fit Note for less than 14 days

If you have been ill for 14 days or less you don’t need to see a Doctor. You can complete a Self-Certification form yourself. However some employers insist on a Doctor’s note regardless. A Doctors note under these circumstances involves a charge of £20.

If you have been issued with a sick note by your doctor and find you are still unfit for work please make a telephone appointment to speak to the doctor.  Please remember to book your telephone consultation in plenty of time for you sick note to be issued. Please note that sick notes can be backdated. 

Doctor’s Sick/Fit Note for more than 14 days

If you have been ill for more than 14 days you will need a doctor’s certificate. You will need to discuss this with the GP over a telephone consultation.

Please book your telephone consultation in plenty of time as these slots can book up quickly! 

 

Requesting an additional Sick/Fit Note

If you have already had a Sick Note for this illness your Doctor will still need to speak to you before they issue. This can be done over the telephone.
